Contest Winner Form

By filling in the below form, I, for good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, do hereby:

i. confirm my compliance with the Official Contest Rules and Regulations (“Official Rules”);

ii. confirm my acceptance of the prize as offered;

iii. release the and hold harmless the Sponsor (named below) and Mommy Connections Inc. and each of their respective agents, employees, directors, successors, and assigns (collectively, the “Releasees”) with respect to any injury, accident, loss or damages relating to the Contest or the awarding of the prize and indemnify the Releasees, their respective agents and the prize suppliers against any and all claims, damages, liabilities, costs and expenses incurred in connection with:
a. the Contest and my participation therein;
b. my acceptance, use or misuse of a prize or any portion thereof; and
c. any claims based on publicity rights, defamation or invasion of privacy;

iv. acknowledge the Releasees’s and its agents’, subsidiaries’ or affiliates’’ right to publish and/or broadcast my picture, name, address, voice, statements and likeness, in any publicity or advertisement carried out by or on behalf of the Sponsor in any manner whatsoever, including print, broadcast or the internet, without further compensation other than the prize offered and, without limiting the generality of the forgoing, grant a non-exclusive, royalty free and irrevocable right and license to publish, display, reproduce, modify, edit or otherwise use my Submitted Content (as defined in the Official Rules), in whole or in part to, and waive any moral rights therein in favour of the Sponsor;

v. acknowledge that Mommy Connections Inc. is not responsible if Contest cannot take place or if any element of a prize does not take place or cannot be awarded or used due to reasons outside of the Sponsor’s control, including weather;

vi. acknowledge and agree to sign such further documents as may be necessary, as determined by the Sponsor in its discretion, in relation to my participation in the Contest, including relating to the use of my social media personal profile(s), without further compensation other than the prize offered; and
