What is Posterior Labor? By Dr. Joelle Johnson

10/06/2011

During labor, signs of posterior baby (sunny side up, spine against spine) can be that early labor may be marked by a long period of irregular uterine contractions with little or no dilation and consistent low back pain.  Contractions may be more frequent and short duration The mother usually complains of a persistent backache, which even in early labor may be severe enough that the pain of contractions is secondary.

It is relatively simple to assist the rotation of the baby when the mother is in early labor. If a baby is in a posterior position, the first thing to do is have the mother get on her hands and knees position for approximately 45 minutes.  Although this position is not the most comfortable one for the mother, it is very effective as it allows the baby more room in which to rotate.  Make sure that she is well-supported by lots of pillows and give her lots of encouragement and emotional support.  Often, while in the hands and knees position the contractions become more regular and more effective, which also assists the baby to rotate into a better position.  A chiropractic adjustment –specifically to the sacrum, pubic bone and pelvis at this stage in labor can help to open up the pelvis and aid in rotation of the baby as well as reduce the pain of ’back labor’.

If the mother cannot handle the hands and knee position for as long as necessary to turn the baby, lay her on her left side, two pillows under the right knee, which is jack-knifed, left leg straight out and toward the back. This position can also let mom relax and rest.

The following movements can help the baby move through your pelvis, if the baby is not in the best position

– In early labor, walk up stairs, – sideways if you need to( even 2 at a time)

– Rock hips from side to side

– Lunge position with one foot elevated on a chair

– Use kneeling or all-fours positions. Kneeling on one knee can help

– March or ‘tread’ on the spot

– Step on and off a small stool

A posterior baby can be corrected with positioning of mother in labor, but it is easier and more effective if done before labor begins.  This is why Optimum Fetal Positioning should be a lifestyle for you, for those last few months of pregnancy. The only person who can get your baby into a good position is you, and unfortunately, you are going to have to do the work to make it happen! An ounce of prevention is worth a pound of cure! Through early diagnosis and appropriate intervention, many women can prevent long and difficult labors.
